Macarthur Disability Network


Uniting people and acting on quality of life issues

 

Macarthur Disability Network unites people with disabilities, carers and service providers to identify, discuss and act on quality of life issues.

The Network works towards raising community awareness of people with disabilities and improvement of services.

Meetings are held every 6 weeks at different locations throughout the Macarthur region.

The Network encourages community feedback from people with disabilities and their carers/advocates to improve coordination between services.

The Network provides a forum as an instrument for the community to lobby government and non-government bodies for improving and providing new services for people with disabilities and their carers

 Unmet Need Notification Form

 

This form can be used as often as you need to record any unmet service need that identified.

Who can use this form?

  • Service providers (disability and mainstream)
  • People who have a disability
  • Carers, family and friends

What should it be used for?

  • When a service or facility can not be found to meet a person’s need for example.
  • Unable to secure advocacy service, accommodation, day program and/or respite, childcare (including vacation and after school care), access to services provided by any community or government agency.
  • When a person is placed on a waiting list.

How is this information used?

This information will be collated and used for disability services and community planning.  The privacy and confidentiality of any person who is not a service provider will be maintained at all times.

Providing personal details is optional only. If this information is provided, it may be used to contact the person for clarification of a point, or to give up-dated service information. Personal information will not be given out unless consent has been given.
