Mental Health Support Coordination

Our experienced Support Coordination staff work with you to contact organisations, community groups and services to research your options and obtain quotes so you can make an informed decision about which services you would like to purchase with your NDIS funding. After which we will coordinate the process of signing service agreements and booking your services.

We are here to make the entire process seamless and stress free for you.

Support Coordination also encompasses a capacity-building component. This is where our staff will teach you the skills and help you to develop the confidence to manage your own supports in the future. The capacity building is conducted via practical workshops; one on one mentoring and written materials.

NDIS Mental Health Support

This service offers individual support to participants with a psychosocial disability to work towards their NDIS goals with a focus on recovery from a mental illness.

Supports participants to improve their daily living skills, empowering them to become more independent. Daily living skills may include cooking, budgeting, shopping, social and community participation, recreational activities, life management.

Continuity of Support (COS)

This service is funded by PHN (Primary Health Network) to provide support to previous Partners in Recovery and Personal Helpers and Mentors participants who were found to be ineligible for the NDIS.

The program’s focus is on establishing and running group activities and offering one on one supports during mental health relapses and/or crises. Some of the groups include; Training Wellways, Social engagement (different outings), Cultural awareness, Movies and discussions, Walking Group, Coffee Club

This service is not open for new referrals.

 

National Psychosocial Support- Transition

This service is funded to provide support to previous Partners in Recovery and Personal Helpers and Mentors participants who have a goal to transition to the NDIS.

Includes advocating with medical professionals and allied health on behalf of the participants to help them obtain evidence which meets the NDIS requirements.

This service is not open for new referrals.

 

Recovery Coach Support

A Recovery Coach is a Mental Health focused, experienced and trained worker who supports people with a psychosocial disability to live a full and contributing life by working alongside each individual providing assistance that builds capacity and resilience through strong and respectful relationships. The Recovery Coach’s approach is based on the Recovery Model which consists of supporting individuals on their recovery journey by providing support through challenging emotions and empowerment to progress towards goals. MDS Recovery Coaches work collaboratively with participants, families, carers and other services to identify, plan, design and coordinate NDIS supports.

What can you expect from a MDS Recovery Coach:

  • Development of a recovery-enabling relationship
  • Supporting your engagement with the NDIS
  • Coaching to increase recovery skills and personal capacity, including motivation, strengths, resilience and decision-making
  • Supporting you with your recovery planning
  • Collaborating with the broader system of supports

Meet Lesina

Lesina had a work accident 2 years ago and as a result of her disability could no longer work. However she was still required to look for employment by Centrelink. This caused Lesina severe anxiety and put a lot of pressure on her to meet the requirements.

With the MDS Mental Health Team’s support, Lesina was able to go through the processes of applying for a Disability Pension to which Lesina was found eligible. Lesina now has less stresses and is in a better financial situation.

The MDS Mental Health Team is currently supporting Lesina to go through the NDIS application process so that she gains access to services that can improve the quality of her life and her independence.
