MDS Club is a social club for participants wanting to participate and attend fun and exciting activities and excursions in the community, at various venues, at different times throughout the week including weekends but not including public holidays. This is a great opportunity to meet up with existing friends and make new ones.
Yes, it is free to become a MDS Club Member. Activities and excursions are at a cost to the participant.
Go to the MDS Club website (www.mds-club.square.site) or click through via the MDS website and click a JOIN NOW button. You will be asked to complete the MDS Club Registration Form. Once submitted you will be contacted by an MDS staff member who will discuss your requirements, this will then lead to the development of a Service Agreement.
After you have joined the MDS Club, you will receive a Welcome gift which will include; a backpack, drink bottle, MDS Club t-shirt, hat and MDS Club Member photo ID card.
The staff support will be in accordance with your current support ratio’s which can be taken out of your NDIS plan. For higher support needs, staff support is dependent on availability.
Your Service Agreement will outline how many hours you wish to allocate to MDS Club and how many staff supported transport hours and kilometer usage; to and from the allocated meeting point.
You must be a MDS Club Member before you can book an activities or excursion. Once you are a member of the MDS Club, you can book and pay online via the MDS Club website, all activities and excursions will be listed. On the MDS Club Website you will register your interest and pay the combined activity/transport cost to confirm your booking.
*If an event is cancelled by MDS you will be refunded your money, if an event is cancelled by you, you will incur the costs.
Yes, you will be provided a Membership Code Word and will be asked for this code when confirming you booking. The Membership Code Word will be provide in the MDS Club newsletter and flyers each month.
If MDS does not already have, MDS will require a current Client Profile.If you take medication, you will require your GP to complete a medication consent form along with pharmacy form and medication dispensed in a webster pack. If not already provided to MDS, you will need to provide relevant management plans e.g. seizure management, asthma management, behavior support plan, mealtime management, manual handling plan etc.
It is up to you whether you bring spending money. On some excursions and activities meals may not be provided so you may wish to bring money to buy a meal/drink/ice-cream etc.
The activity/excursion cost is inclusive of venue/ticket/entry cost, transport to and from and may or may not include a meal. See description of each activity/excursion for meal inclusions.
Please note that not all venues will be suitable for participants with mobility issues; any accessible restrictions will be noted on the MDS Club website booking form. If you are unsure please call a MDS staff, details listed on website for further information prior to confirming your booking.
Be rewarded and accumulate loyalty points for attending your favourite activities and excursions. If you attend 10 activities, on the 11th activity, you will receive a free meal voucher.
This voucher will be provided to you by an MDS staff member.
Yes! You can invite your friends, spread the word but they must JOIN MDS Club before booking an event or activity.

Yes, we want to hear your thoughts and feedback, what your interests are, where you would like to go and who you want to go with. MDS will not always be able to meet every need but we will do our best. MDS Club is about YOU a MDS Club Member.
